The file below is the application form that needs to be completed during the application procedure for working with children, young people or vulnerable adults.

Please download, print, complete and sign the form. Upon completion please submit the form to the church office:

You will be required to provide a valid DBS report and attend an meeting with the Safeguarding Co-ordinator and another (usually the group leader you are applying to volunteer for) prior to being approved for working with children, young people or vulnerable adults at Living Waters Church.
